2023년 11월 10일부터 12일까지 열렸던 PRS KOREA 2023 국제 학회에서 일퍼센트성형외과 임종우 대표원장님이 “Bespoke Concept and Strategy of Facial Contouring Surgery” 라는 제목으로 발표를 하였습니다
특히 이번 발표에서는 안면윤곽수술에 있어서 맞춤 디자인의 중요성을 강조하기 위해서 일퍼센트성형외과에서 고안한 “얼굴형 MBTI” 를 처음으로 소개하여 국내외 성형외과 전문의들에게 “참신하고 유용한 얼굴형 진단 Tool” 이라는 코멘트를 많이 받았는데요. 최근에 이 얼굴형 MBTI 가 특허청으로부터 정식으로 상표등록증을 받아서 더 자랑스럽게 생각합니다.

이 얼굴형 MBTI는 누구나 쉽게 자신의 얼굴형을 자가 진단해볼 수 있도록 임종우 대표원장님이 순수하게 자신의 아이디어로 개발한 검사방법인데요.
마치 누구나 한번쯤 해보았을 ‘성격유형 MBTI’ 와 비슷하게 4가지 영역으로 나눠져있습니다.

- 연부조직의 비율(Fat)B(Bone)/ F(Fat)

2분 내외로 간단한 설문에 응답만 하시면 이 4가지 카테고리에서 2가지 중 하나로 결정이 되어 총 16가지의 얼굴형 중 하나로 분류되는데 마치 성격유형 MBTI 가 ESFJ, ISTP 등 16가지로 나오는 것과 유사하여 “얼굴형 MBTI”라고 부르기로 한 것입니다.
임종우 대표원장은 첫 상담을 오시는 모든 분들에게 이 얼굴형 MBTI 를 해보도록 권유하고 이를 바탕으로 상담시에 본인의 얼굴형에 대해서 정확하게 알고 있는지 아니면 다르게 알고 있는지 부터 상담을 시작하고 있습니다.
이 4가지 카테고리로 분류를 한 이유는 그에 따라 수술 계획도 달라지기 때문인데요. 예를들어 가로/세로 비율이 넓고 짧은 WS 형과 좁고 긴 NL 형은 윤곽수술 플랜이 전혀 다르게 되고, 뼈가 우세한 B 형과 살이 많은 F 형 역시 뼈수술만으로도 효과가 좋을지, 지방흡입이 추가로 필요할지 계획이 달라집니다. 피부 탄력이 좋은 T형과 그렇지 못한 W 형 역시 부가적인 리프팅 수술이 필요할지 결정하는데 많은 도움이 되겠죠?
이렇듯 자신의 얼굴형에 대해서 먼저 자가진단을 해보고 전문가인 원장님과 상담을 받으면 자신이 잘못 알고 있었던 부분은 3D CT 를 바탕으로 전문적인 진단을 받으면서 제대로 인지하게 될 수 있고, 원장님의 수술 플랜에 대해서도 쉽게 이해할 수 있다는 장점이 있습니다.
이렇듯 다양한 얼굴형에 대해서 정확하게 진단한 이후에 필요한 것은 개개인의 “선호도”에 대해서 귀담아 듣는 것인데요. 왜냐하면 똑같은 얼굴형을 가지고 있더라도 원하는 얼굴형에 대해서는 전부 다른 선호도를 가지고 있기 마련이고, 반대로 전혀 다른 얼굴형을 가지고 있더라도 같은 얼굴형을 원하는 경우가 비일비재하기 때문입니다.

그렇기 때문에 임종우 원장은 획일적인 수술방법이 아니라 현재의 얼굴형과 원하는 얼굴형을 1대1로 매칭시킬 수 있는 맞춤(Bespoke) 수술기술이 중요하다고 강조하였고, 바로 이 부분이 일퍼센트성형외과에서 비스포크 안면윤곽수술이라는 이름을 붙인 이유랍니다.

Facial contouring surgery is very popular in Asia. In early days, facial contouring patients wanted just small and slender face as much as possible. Nowadays, however, individual patients tend to persue different facial contour according to their various preference.
For example, some patients wish to have very sharp and narrow jaw line, so-called V-line, while other patients wish to have round and natural jaw line like U-shape. Some patients want their malar shape to become straight and flat, while other patients want their malar shape to be curvy and volumetric. As the proverb says, beauty is in the eye of the beholder, individual preference on facial contour should be respected by surgeon. If a surgeon insisted on certain beauty concept and didn’t listen to patients’ needs, the surgeon would frequently encounter dissatisfaction from patients despite best effort on facial contouring surgery procedures.
Therefore, surgeons should pay attention to their patients’ preference on facial contour and be ready to perform different procedures to fulfill various needs. For instance, a single osteotomy method such as T-osteotomy or inverted V-shaped osteotomy in genioplasty cannot meet every patients’ desires. Because preference on chin shape varies from person to person, surgeon should customize the genioplasty method to match the present shape with desired one.
So I propose bespoke concept of facial contouring surgery to satisfy individual patient. In this presentation, I will introduce my personal strategies of facial contouring surgery in detailed customized manners.
