9월 4일에 열렸던 2021년 대한두개안면성형외과학회 추계 학술대회에서 임종우원장님께서 "Aesthetic soft tissue management in malarplasty: Intraoral midface lifting using Endotine" (애플존 광대수술) 라는 주제로 강의하셨습니다.
2. Aesthetic soft tissue management in malarplasty

안면윤곽수술을 주로 하는 임상의로서 광대축소수술을 할 때 얼굴뼈 만큼이나 중요한 것이 연부조직의 변화라는 것을 깨닫는데 그리 오래 걸리지 않았습니다. 환자마다 피부탄력, 볼살의 무게 등이 다르고 광대축소수술시 절골하는 뼈의 크기와 광대 체부의 setback 정도에 따라서 연부조직의 변화는 다양하기 때문에 미용적으로 보다 만족스러운 광대축소수술을 위해서는 이런 연부조직의 변화를 제어하고, 경우에 따라서는 보완하는 추가 수술이 필요하다고 생각합니다.
광대축소수술후 볼처짐의 가능성이 높은 환자군에서 저의 윤곽수술 초창기에는 아큐스컬프를 이용한 지방흡입과 실 리프팅을 병행하는 방법을 사용했었는데 결과가 만족스럽지 않아서 이후 다양한 시도를 해왔습니다.
특히 Malar fat pad 가 내려와서 팔자주름이 이미 깊어져 있는 환자군에서는 엔도타인을 이용한 중안면부 거상수술을 변형시켜서 입안 절개를 통해서 광대수술과 동시에 시행하고 있는데, 기존의 실리프팅보다 효과가 좋고 고전적인 안면거상수술보다 간편하여 수술하는 의사와 환자 모두에게 좋은 수술법이라고 생각합니다.
안면윤곽수술을 하는 의사로서 얼굴뼈 뿐 아니라 연부조직에 대해서 계속 고민하면서 다양한 연부조직 수술법에 관심을 가지고 새로운 방법을 시도해보는 것이 중요하다고 생각하여 저의 경험을 여러분과 공유하고 싶어서 이번 발표를 준비하였습니다.

Indication
High probability of drooping during zygoma reduction / Hollowness of anterior malar causing old-looking appearance / Deep nasolabial folds because of malar fat pad descent
Midface lifting using endotine
Subconjunctival or subcilliary Incision – subperiosteal dissection- Endotine Midface Insertion – soft tissue hanging – lifting - fixation (1. 결막내 또는 피부선 절개 → 2. 골막하 박리 → 3. 엔도타인 미드페이스 삽입 → 4. 골막층 고정 → 5. 뼈에 구멍을 만든후 나사로 고정한다. → 6. 남는 부분 제거)
Modification for intraoral approach
Intraoral incision – subperiosteal dissection – L-osteotomy – fixation with plate & screws – Endotine insertion and fixation on plate – lifting sagged soft tissue to obliquely upward direction – hanging the tissue to Endotine by digital press – evaluation of lifting effect and readjust if needed (엔도타인 삽입 후 고정 → 살을 위로 끌어올려 리프팅 후 고정)
Expected result
Prevention of drooping after zygoma reduction / Augmentation effect of anterior malar leading to younger looking / Lifting malar fat pad leading to Improvement of deep nasolabial folds
CT postop. 4 month
In my experiences…
No need of additional incision : short op. time / Simple and easy to apply : short learning curve / Reliable effect : short leverage distance / Reasonable cost compared to other lifting procedures / Low risk of complication than expected : palpability, numbness, hypesthesia, seroma, infection, hematoma / Cautions – Careful application not to irritate the infraorbital nerve : both fixation and hanging
Summary
Soft tissue management and adjuvant procedure are important for facial bone surgeons to achieve aesthetic satisfaction from various patients / Zygoma reduction with midface lifting using Endotine is a good option in certain indications. / It is a simple, effective, and safe procedure if performed carefully. / Cost-benefit to both surgeon and patient
